Up-to-date immunizations
Eye Exam
Physical (on a Kentucky form)
A regular size backpack
Fitted crib sheet
Blanket or towel to cover up with at rest time
Snack and drink daily (juice or water for the drink and we ask for a healthy snack-NO candy, cakes, or soft drinks)
Change of clothes (including underwear and socks)
Diapers/Pull-ups if not toilet trained
